INSURANCE PLANS
We accept most major insurance plans, however you should always check with your insurance plan prior to your visit. We do not accept any crossover plans, Ambetter, United Healthcare Duel Complete.
If you have any questions regarding your plan, you may contact our billing department at 901-753-2794 and select the option for billing.
MEDICAID
We only accept Tennessee BlueCross BlueShield’s BlueCare and Tennessee Crossover plans.
We do not accept Tenncare plans administered by United Healthcare (Amerigroup or Community Plan) or any Tenncare plans administered by any states other than Tennessee.
COVID-19 RESPONSE
COVID-19 UPDATES
Masks are optional in our office per CDC recommendations. If you have symptoms, such as, cough, fever, sore throat, or cold related symptoms, or have been exposed to someone with COVID within the last 5 days, you are required to wear a mask in the office. Due to skin exams being within close contact many patients may prefer mask for their protection and ours.
Re-scheduling for patients not feeling well
If you are sick or have had a fever, cough or shortness of breath and have tested positive for COVID within 5 days, we ask that you re-schedule your appointment.
PATIENT FORMS
In any medical treatment, the first step is information-gathering. The more we know about you, your personal and family medical history, your current condition, and your current medical and drug therapies, the better we will be able to diagnose and treat your symptoms. That’s why we ask you to fill out digital patient forms when you come to see us. Upon making an appointment, our staff will provide digital forms to the email address you provide.
Please bring your ID, insurance card, and copayment for your appointment.
